Skilled Nursing Care:munity Care and Rehabilitation Center provides complete nursing services to meet the needs of post hospital care and rehabilitation, as well as continuing care for our long term Residents. We serve both short and long term Residents under the direction of the attending physician. Our staff of dedicated professionals have years of experience in providing care for patients and residents of all ages and medical conditions. Plans of care are developed in concert with the attending physician specifically for each individual and implemented by our health professionals. Whether the individual’s needs are for rehabilitation, sub-acute medical or supportive care, we pride ourselves in fulfilling those need for every person we serve. Available Services Include:

Rehabilitation Services 

Our outstanding therapy program is well known for its success in restoring lost abilities after an injury or illness. Physical, occupational and speech therapists work daily to evaluate and educate patients and their families to enable them to make a successful transition back to the home environment. Respiratory therapy is also available when needed. Our program is aggressive yet sensitive to the special needs of older patients. A qualified Social Services staff assists in developing plans for care at home and other post discharge services. Psychological support services are also available to address the particular needs of our patients and residents. Regular exercise programs are planned for long term residents to maintain and improve their mobility and well-being.

Social ServicesOur Social Services Department provides support for our Residents in helping them adjust to a new living setting or planning their transition to home after their stay with us. They also assist Residents with getting the support and services they need to make their lives more meaningful however long they stay at CCRC

Dietary Services

Under the supervision of our Registered Dietician our Dietary Department Staff prepare tasty and inviting meals that are carefully planned to conform to the many special diets that our Residents require. Each Residents’ diet is customized to their requests and restrictions. Snacks and supplements are provided throughout the day. Alternate entrée choices are available at all times. Meals can be served in the Resident’s room or in one of our attractive dining rooms.

Hospice Care

CCRC offers dignified and comfortable hospice services for the terminally ill. In concert with outside Hospice Services our staff provides supportive care as well as the medical support the patient and their family needs at a difficult time.

Amenities

Community Care & Rehab is beautifully appointed in a residential style. It is designed to create a “home away from home” regardless of whether the patient stays for a few days or several years. Comfortable lounges, dining rooms and open patios are found throughout the Center. Our beauty parlor is staffed with a licensed beautician and provides a full array of services. Every room has its own television with bedside remote and pillow speaker. Cable TV and telephone services is available as an added service. Each room has an inviting patio which is a welcoming and comfortable space.
