Freedom Square Rehabilitation and Nursing Services (FSR) received the highest possible overall rating of 5 of 5 stars based on the Centers for Medicare and Medicaid Services (CMS) rating system. The Medicare.gov Nursing Home Compare web site reflects the latest ratings.

The web site has detailed information about every Medicare and Medicaid-certified nursing home in the country based on health inspections, staffing, and quality measures. Currently, there are 28 nursing homes within 10 miles from the center of Seminole and FSR was one of only two with an overall rating of 5 stars.
