We understand that seniors with Alzheimer’s and dementia benefit from maintaining familiar daily routines. Therefore, we take a personalized approach in providing care, one that takes into account physical and behavioral needs, as well as life history. This allows us to create an individualized service plan designed to meet each resident’s unique needs, while also helping them feel safe and add meaning to their day.

Through the Clare Bridge Daily Path program, we provide a gentle structure to each day, one that includes:

• Morning Mental Workout – Daily late morning mental exercises including discussions, brain stimulating games, and reminiscing.
• Daily Physical Activity – A variety of exercise opportunities including group exercises, walking programs and dancing.
• Daily Life Skills – Individual or group activities that provide a sense of purpose, meaning, and belonging.
• Person Centered Life Enrichment Programming – Activities are designed using the resident’s own social history information as a foundation.

Brookdale Troy also features peaceful surroundings and a home-like setting designed to re-create environments that seniors have enjoyed throughout their lives. Interior spaces are scaled and decorated like those of a private home. Enclosed walking areas give residents the opportunity to safely explore their surroundings.

The Clare Bridge Dining Program, recognized as “Best of the Best” in the industry by the Assisted Living Federation of America (ALFA) is a unique program designed by a team of specialists to help those with Alzheimer’s and dementia master and enjoy successful dining experiences. It offers dementia-friendly menus, individualized dining assistance, a supportive environment and consistent seating arrangements.

Our community is located next to Brookdale Troy, our sister property, so that we can provide a full continuum of Personalized Assisted Living and Memory Care for seniors.

Prior to moving in, our staff takes a social history on every resident to get to know their individual likes and dislikes. Programs and outings are specially designed with residents’ individual interests and abilities in mind. Multiple programs are offered throughout the day and evening hours. These programs are designed and supported by our dementia care specialists.
