Autumn Care of Nash is the only locally-owned and operated nursing and rehab facility in Nash County. Conveniently located to hospitals in Rocky Mount, Tarboro, Roanoke Rapids, Wilson, and Raleigh, our 80 bed facility is nestled in a beautifully landscaped and wooded setting. Residential amodations include private and semi-private rooms that are tastefully decorated and offer scenic views of the grounds and garden areas. One three-room suite is available to individuals that enjoy entertaining in their own private living room with kitchen amenities such as microwave, refrigerator, and sink. A full bathroom, including easy accessible shower is conveniently located to the suite’s private bedroom. Many residents have personalized their rooms with mementoes from home such as pictures, floral arrangements, lamps, and favorite chairs. Residents can entertain family and friends in the spacious lobby, music room with aviary, television room/library, inner courtyards with benchesand bunny habitat, or one of five covered porches with rocking chairs. A private dining room is offered for special family events such as birthdays, anniversaries, or family reunions.

We offer sixty beds which are certified by Medicare for skilled nursing services and short-term rehabilitation. Twenty beds are designated for rest home services and are often utilized for short-term respite stays. Registered and licensed nurses provide 24 hour coverage. Our highly-trained certified nursing assistants render care and assistance in bathing, dressing, eating, toileting, and transferring. Autumn’s rehab staff provides physical, speech, and occupational therapy to those individuals recovering from surgery, stroke, or debilitating illness. The rehab staff recently acquired new modalities/equipment for facilitating muscle strengthening, pain management, and wound healing. In the past year, the majority of patients admitted to Autumn Care of Nash were discharged home or to an assisted living facility. 

Through the development of individualized care plans and treatment programs, residents are encouraged to be as independent as possible. The multidisciplinary care plan team meets with family members and residents frequently to keep them apprised of the resident’s progress in attaining his or her goals.

Residents are encouraged to participate in various activities and discover new interests and talents. Our dedicated Activity Department posts a monthly calendar of events that include bible study, exercise programs, music therapy, movies, manicures, pet therapy, dances, dining out, and shopping excursions. Many residents enjoy less structured activities such as watching our resident finches and rabbits. A dedicated group of volunteers spend time on an individual basis with residents to read aloud, assist in letter writing to family and friends, or just engage in friendly conversation. 

Autumn Care of Nash is proud to be a teaching site for various programs offered by Nash Community College, Edgecombe Community College, and Nash County school system. Nursing, nursing assistant, and rehab students complete the clinical portion of their studies working closely with our staff and residents.
