The Willows Nursing & Rehabilitation Center is a leading provider of long-term skilled nursing care and short-term rehabilitation solutions. Our 57 bed facility offers a full continuum of services and care focused around each individual in today’s ever-changing healthcare environment. 

The Willows Nursing & Rehabilitation Center has been serving Dane, Jefferson and Columbus Counties since 1970. We are located on the northwest side of Sun Prairie, situated in a quiet, lovely residential neighborhood. Several churches, shopping centers, restaurants and a park where summer music concerts are played are just minutes away. St. Mary's Hospital and Meriter Hospital are within twelve miles of our facility. Our facility is Medicare and Medicaid certified.
