Heritage Health - Streator was built in 1964. Our facility was expanded in 1968, and we are currently a 130-bed facility, providing therapy, hospice care, and respite care, among other services.

In addition, we staff a Skilled Nursing Service in-house, 24 hours a day, 7 days a week.

Residents also enjoy many activities, 7 days a week, both within our facility and within our community. This is just one more way we make our residents feel at home.
