At In-Home Care we help people live full, independent, and dignified lives in thefort of their own home. We are dedicated to providing care that enriches life and helps maintain the highest possible level of independence and safety. In-Home Care is a licensed NH Home Health Care agency that offers comprehensive, personal care services to elderly and disabled people in NH. Our consumers use either the Medicaid Choices for Independence program or are Private Care. Please read below to determine the type of service that is right for you.

Private Care Support Services include:

• Light housekeeping and laundry
• Bathing and personal care
• Meal assistance
• Medication reminders
• Transportation to appointments
• Exercise, as instructed by your doctor, nurse or therapist
• Companionship
• Recreational activities
• Mail assistance and organization

It's your choice! You will meet and approve your caregiver before service begins. Our caregivers are people you can trust; many clients consider them as part of the family. Caregivers have the training, personality and availability to assist you. They are pre-screened, bonded and covered by Workers' Compensation Insurance.

In-Home Care is the provider of choice for many NH residents who are eligible for the Medicaid "Choices for Independence" program . We are a Licensed Home Health Care agency.

With self-directed In-Home Care you can:

• Choose and manage the person providing the nonmedical home-based caregiving services you need. You can recommend a friend or family member to provide your care, or we can supply you with a list of pre-screened caregivers who are ready to assist you.
• Determine what (allowable) community-based services best meet your needs.
• Assess the quality of services received and make changes based on your needs.

Our services allow you and your family to play an important role in the selection and management of your caregiver - who can even be a family member or friend. Our role is mostly administrative. In-Home Care handles the administrative aspects of your caregiver's employment (pre-screening, time sheets, payroll, tax-related issues, etc). You manage your caregiver, including all scheduling and backup contingencies. As a participant, you will also have access to In-Home Care coordinators for support. We are here to manage your caregiver over time, and will also check in with you regularly to see how things are going.
