North Carolina Assisted Living Facility Over Sight

North Carolina assisted living facilities are inspected by Department of Social Services for each county. Full assessments are done annually and facilities are monitored on a quarterly basis. To check the status of an assisted living facility including copies of inspection report and to check complaints, call the adult care Licensure Section of the Department of Social Services at 919-855-3765. To file a complaint, call the Complaint Intake Unit at 919-733-8499. You may also contact your North Carolina Ombudsman to get assistance with problems at an assisted living facility.
