To file a complaint or get a copy of existing complaints for a specific facility you can call the Department of Health Licensing Agency at 877-287- 0010. You may also call long-term care ombudsman at 877-236-0013. Even if you have a family member or a local advocate to help you with your assisted living problems, the Tennessee ombudsman's office is available to help seniors with any problems associated with a Tennessee assisted living facility.
