The Oregon Department of Human Services, Seniors and People with Disabilities Licensing and Quality of Care is responsible for inspecting assisted living facilities.  They perform inspections every two years and post the inspection reports at the facility.  You may also call them  to request copies of inspection reports or copies of complaints at 503-945-5832 or 800-232-3020. To file a complaint, contact  the local office of the Area Agency on Aging Seniors and People with Disabilities. You may also want to contact to your Oregon Ombudsman:
